I prefer the title Ms.
I preferred Ms. to Miss when I was still single and using my original surname.
I prefer Ms. to Mrs. now, when I have been married for just shy of six years, have used my husband's surname for nearly all of that time, and borne him two adorable and goofy sons.
Yes, Ms. can be used with a married name as well as one's original name. No, you don't have to use Mrs. if you change your surname upon marriage. There would be no point to having a title that is as neutral to marital status for women as Mr. is for men if it were only used with your original surname or only by unmarried women.
I don't care what title or surname other women use, or what their marital status is. All I ask is that I be kept informed of preferences and changes so I can continue to use the correct form.
