Fun with medical claims

Mark's second appt. with Dr. Hatfield's practice was originally slated for October 8th. We were promised to get a call if an opening came up sooner, and a little over a week ago, they did just that.
First offering was October 1st. That was a no-go, as it's school headcount day. State funding for the year depends on how many students are physically present. Not how many are enrolled, or students present plus students with excused absences. Only students physically present.
Esp. in a cash-strapped district like ours, this is kinda important.
Second offering was for tomorrow, either 9 or 10 am. I chose 9 am. Everything seemed fine.
Thursday I get a call from Dr. H's office staff, saying they need an ECHO authorization form, and can I do anything about getting it faxed to them. I said they'd have to talk to Ms. Willers, our ECHO co-ordinator, as she'd have that information; but if they didn't hear from her/didn't get the form by noon Friday, to please call me back and I'd see if I could reach Ms. W. I didn't want to fail to have all the paperwork in hand before Tuesday's appt.
Didn't get a call Friday. Assumed all was well.
Got a call today around 2, from a diff. member of Dr. H's office staff. She had just gotten through her pile of messages from last week, and found the note saying I was to be called back if they didn't have the ECHO form. She said she couldn't find the form, so assumed it hadn't been sent, and would I please contact Ms. Willers?
I swore internally, and got the specific name of the form, their office number, and their fax number.
Called Ms. W an hour later, due to Acts of Zodling.
Ms. W was deeply displeased. She sent them the form back on Thursday. And had gotten a call from a staff member (Apparently completely different from the one I spoke to last week *and* the one I spoke to today) regarding this. She says she's sent them every single form they need, and that their requests to find out whether a therapist in Dr. H's practice is included in the forms which refer to Dr. H by name need to be sent to Billing, not to her. Which she's told them many times. She says she doesn't usually have this much trouble dealing with staff members of her clients' doctors.
Me? I'm getting a little annoyed myself. If Dr. H's therapists do great work with Mark, we'll stick around. But if they only do mediocre work, and esp. if this office staff nonsense keeps up, we may need to shop around for a different practice.
